Directory of Services- support related to accommodation

The approach of the council's staff, social workers and other care managers, is to assist people with social care needs to recover and return their own home. More details are can be found on our support for older people page.

Sometimes after services to assist their recovery, accommodation cannot be made suitable for an older people with disabilities. Then a range of alternatives are available, that are especially adapted, and can include arrangements for personal care, meals and other support on an individual basis according to a care plan.

In Coventry, with respect to people who do not have high nursing needs, there is a strong policy, where appropriate, to offer Housing with Care alternatives, to enable the person who needs support to have more independence and control.

Coventry's Community Plan reflects this, and it follows the Council's Housing Strategy, which has a mission statement "to ensure decent homes, housing choice and support for Coventry Citizens". More details on the policy are available on this page about housing with care for older people.

New developments

There are several new Housing with Care developments. This form of service is sometimes referred to as "Extra Care Housing", and was formerly known as "Very Sheltered Housing". In Coventry these are at present mainly available as tenancies rather than for purchase.

Residential home care and nursing home care also provide accommodation and personal care for older people with disabilities. They are now called respectively "Care Homes" and "Care Homes with Nursing".

If you have health related issues

In most situations ongoing nursing needs are met through the District Nursing service.

If ongoing nursing support is needed which may need to be available with the accommodation, then your social worker or care manager will discuss this with you.

If your needs are similar to those below, a place in a Care Home with Nursing (Nursing Home), which employ qualified nurses as part of the staff may be discussed with you.

Nursing care is needed above a certain time input per day
The nursing care is variable - e.g. it may be needed some nights and not others
Your care needs change and they need to be supervised by qualified nurses

In most situations ongoing nursing needs are met through the District Nursing service. If your need is primarily a healthcare one or you are advised your support could be through a nursing home, you should also have assessments from Coventry NHS Primary Care Trust staff. They will discuss your needs with you and also work out if they can give financial assistance.

Your Community Nurse will initiate this, or if they are not involved, you or your social worker/care manager can approach the the Continuing Healthcare and Adult Assessment Team (CHAAT) and ask for assessments relating to Continuing Health Care and Residential Nursing care Contribution (RNCC). The CHAAT team number is 024 7684 4111.

The following are the most commonly used reources in Coventry. Please note that there are registration categories for some types of need e.g. dementia related care or a care home with qualified nurses as part of the staff. If a care home can meet your need the assessment should clarify the category needed. Subject to introductions and your agreement to the care plan, a financial assessment and contract, an assessment and response team or community mental health team will confirm arrangements.

Even if you have sufficient resources to make your own arrangement you are still advised to seek advice from the assessment and response team first.
