Social care support for adults
Social Services provides people between the ages of 18 and 64 with personal support and services to meet eligible needs. Social services works together with health and other organisations from the voluntary and private sector so that adults within Coventry maintain or improve their independence.
Specialist teams in the city assess individual peoples needs, and provide and arrange services to meet eligible needs. Health services can also be accessed in some of the teams. Anyone can contact them to discuss their requirements or they can ask a relative or friend to do this for them. The teams may also receive a referral through another source such as a GP, but people's permission will always be obtained before this is taken forward. The specialist teams assist the following people:
- People with learning disabilities
- Peoples with physical or sensory impairment
- People with mental health needs
- People with HIV/Aids
- People who care for others
- People who are vulnerable and need protection
- People with drugs / alcohol problems
Services, which are arranged for adults within Coventry, meet their individual needs and this takes into account communication, cultural, religion, language and gender.
When a referral is received someone from Social Services will visit the person to carry out an assessment so that they can gather more information on what type of support or help they may require. This will look at the range of needs that people can have. To get any support or care some of those needs must meet Social Services eligibility criteria.
What happens at an assessment?
We will:
- Talk to the person concerned.
- Talk to family and any carers involved Talk to others who may be involved such as your GP, friends and so forth.
We will only talk to other people or share information we gather if you actually consent to this beforehand and we will always explain how the information will be used and who we are going to discuss it with. Your assessment will not cost you anything.
How do I know if I am eligible for any support or care?
- You will to meet the Social Services eligibility criteria.
- You will have to have needs that meet the critical or substantial criteria.
- We will give you advice about other agencies or resources who may be able to help you if you have needs that do not meet the critical and substantial criteria.
What happens if I think my needs have changed?
- You must tell us immediately.
- We will carry out a new assessment.
- If you do receive a service from Social Services then you will be reviewed regularly.
What happens if I think my assessment was not fair?
- You can ask to have this reviewed.
- If you are still unhappy after this you can comment or complain to the manager of the team who has been dealing with your assessment.
