Transitions

The Children's Disability Team supports young people with a disability aged 15+ transition into adulthood.

Aiming High for Disabled Children (AHDC) is a national transformational programme launched after a Government review of services in 2007. The transformational programme for disabled children's services includes increasing families' choice and control over the support and services they receive via individual budgets, improving and coordinating services for disabled young people in transition to adult life and increasing the quality and quantity of short breaks for disabled children and their families.

We are working with the National Transition Support Team, and piloting Individual Budgets for young people at transition to develop opportunities for:

  • 'try before you buy' opportunities for young people to gain an experience of living independently;
  • person-centred planning to help young people achieve the highest level of independence;
  • assistive technologies such as electronic books and Wayfinder travel training devices; and
  • modern apprenticeships and mentoring programmes.
