PLEASE NOTE: Following a re-commissioning of lifestyle services for adults in Coventry. A range of services have been integrated into a new model to be delivered by ICE Creates. From 1 April 2018 the ‘Be Active Be Healthy’ (BABH) Lifestyle Advisor Service will be moving to ICE Creates following their successful bid for the integrated adult lifestyle service for Coventry. A letter regarding client data transfer will be sent to everyone who is currently accessing the service.

Coventry City Council Health and Wellbeing service is a community based 1:1 support service for anyone living in Coventry who would like to change their lifestyle. The lifestyle advisors have partnerships with a range of community based services and with the 1:1 support will help you to access a variety of opportunities suited to your individual needs.
The following lifestyle interventions can be supported (6 week free programme) which include support around:
Where appropriate the lifestyle advisors will signpost to community based or specialist services to support your lifestyle changes.
Coventry Health and Wellbeing service is a self-referral process. Fill in the self-referral form and someone from the team will contact you.
To be eligible you must fit the following criteria:
On your first appointment a health assessment form will be completed to see what your needs are, the lifestyle advisor will then decide the best intervention for you. These range from 6-12 week interventions depending on what the the best option is. Follow up can be completed up to 12 months on some interventions.
