Education setting identify that a child has a long term SEN/Disability who may require support over and above the notional SEN budget.

Meeting held with family, child/young person and multi-agency professionals at the setting to discuss whether a non-statutory My Support plan or statutory assessment may be required.

Multi Agency Panel makes formal decision for statutory assessment or non-statutory My Support plan within 6 weeks of receiving formal written request.

Not agreed - My Support Plan

Contact made with family to advise of decision and rights of appeal - My Support Plan.

Agreed

My Story, statutory assessment, co-ordinated appointments made with professionals. Reports completed and submitted.

14 - 16 weeks

No EHC Plan required

Meeting with family/setting to discuss assessment outcomes and how support can be met through My Support Plan.

EHCP Plan required

Person centred meeting with family/setting to agree the draft EHCP and outcomes. Family express preference for setting

16 - 18 weeks

LA consults with setting, giving 15 days deadline

20 weeks

Following consultation the LA issues the final Education Health and Care Plan naming educational setting and giving right of appeal
