Improving Lives

​As part of the Adult Social Care Service, we are working more closely with other organisations to improve the outcomes for adults, and aim to: prevent hospital admissions, reduce the length of a hospital stay and set up integrated processes to support discharge from hospital. To achieve this, we are running a project called Improving Lives. It is a joint partnership programme bringing together UHCW, CWPT and the Council to work as one service.

We believe that this approach will lead to increased numbers of adults leading more independent and healthier lives.

"The Improving Lives Programme is bringing partners together to implement an integrated model that focuses on delivering more effective care and improving outcomes for people who require hospital or alternative care. Three sub-programmes have been established; Interfaces, Hospital Processes at UHCW and One Coventry Integrated Team. Delivering the Programme is an identified priority within the Supporting People at Home section of the plan.

Improving Lives is focused on the patient experience and the service user's journey.

The programme hopes to enable individuals to only need to tell their story once. The programme also aims to prevent hospital admissions and help the discharge process from hospital.

A specialist from acute and community teams is in post, reducing the need to refer people in to different services. This enables ease of sharing knowledge.

"It is already working – people are moving through services quicker and we are reducing the need for ongoing services."

This is about putting people at the heart of the services. It is a One Coventry approach involving health and social care agencies.
