Births and death registrations:

I understand that I must check the registration entry very carefully before signing as any errors are subject to a correction consideration fee of £99.00.

I have read and understand the following.

Please bring the following supporting documents to a birth registration:

If you are married or in a legal civil partnership only one parent needs to attend.

If you are not married or in a legal civil partnership with each other, and you wish for the father’s or second female parent’s details to be included in the birth registration, both mother and father/ second female parent must attend the appointment together.
