If you are someone who is heading into adulthood, finishing school or college or even finishing education entirely and you have a care need, we want you to know there is support available.

You don’t have to make those next steps alone.

On Tuesday 10 November you can come to our event anytime between 2–5.30pm at Cheylesmore Community Centre.

You will find out more about the support available from Adult Social Care, local voluntary and community organisations, and health partners.  We’ll be offering advice and information on a range of travel, employment, and independent living advice on offer to help you feel safe and prepared.

At this event you can:

  •  Meet the Transitions Team
  •  Learn about preparing for adulthood
  •  Explore local support and services, including respite and break options
  •  Find out about opportunities for education, employment, independence, and wellbeing

You will be able to ask questions and speak directly with professionals and support organisations. Whether you are a young person, parent, carer, or supporter, this event is a great opportunity to discover what help is available and plan for the future.
