This Privacy Notice explains how personal information is going to be used, what for, who it will be shared with and why.

Why do we collect and use personal data about you? 

We collect the information to inform the healthy lifestyles needs assessment to ensure our provision meets the needs of people in the city.

We use this information to:

  • Help us understand how people use healthy lifestyle services
  • Help us understand what people want in the future to help improve their health
  • Process the prize draw if you choose to enter.

Our lawful basis for processing personal data is Consent.

What personal information do we collect about you?

Survey

The information collected in the survey is held anonymously. The survey data is combined into statistical data.

Prize Draw

If you requested to be included in the prize draw, the following personal information is also required:

We collect and hold the following information about you until the prize draw has taken place.

  • Full name
  • Telephone number

Qualitative interviews

The information collected during the interviews is given below. We will collect information so that we can contact you, but this will be held separately from other personal data you give us. The results of the qualitative interviews will combined together before reporting.

Who do we share your personal data with?

The personal data collected will be anonymised, analysed and separated into ward data and looking at responses by ethnic group, gender, age health and disability to allow us a better understanding where to target our services and support. However, the data will only be statistical data, therefore, specific personal data would not be able to be identified. The anonymised data may be shared within the Council, with elected members and with services that aim to improve health and wellbeing in Coventry so that can better support their service users.

How long will we keep your information?

We will only keep your information for as long as we need to so we can give you the service you need unless we must keep it for legal reasons.

Your rights

You have the right to:

  1. Ask to see the personal information we hold about you
  2. Withdraw consent (see below)
  3. Ask us to change information we hold about you if it is wrong
  4. Ask us to delete the information we hold about you
  5. Ask us to limit the way we use your personal information
  6. Ask for human intervention regarding decisions made about you by a computer
  7. Data portability (have your data transferred to another Authority)
  8. Complain to the Information Commissioner’s Office

Withdrawal of consent

Where personal data is processed on the basis of consent, you will have the right to withdraw that consent.

Complaints

You have the right to submit a complaint if you are unhappy with the way your request is handled or disagree with a decision made by the council regarding your data. In these circumstances, you can contact the Data Protection Team (DPO) and request a review of the decision.
