As a Coventry City Council procured provider, CAP expect you to deal with any low level behaviour traits internally using your organisation policies supported by CAP Policies and Code of Conduct. Our service are always readily available to offer advice, guidance and support.

Guidance for reporting incidents

To give guidance on how the incident reporting process works, we have created a clear process. This guidance should be used as a guide only, we understand that incidents must be reviewed on an individual basis. 

Informal

Incident type

Low level poor behaviour demonstrated or a minor incident has taken place.

Consequence may include;

A verbal warning should be issued, with a reminder of expectations. Reference to the Code of Conduct.

Case 1

Incident type

Repetition of low level inappropriate behaviour / uncooperative / disrupting learning of others.

Consequence may include;

  • Behaviour to be discussed with learner.
  • An internal sanction must be agreed.
  • The Contribution function can be used on CLM.
  • Incident Report - Level 1 to be completed and submitted to CAP.
  • School will receive a copy of this report from CAP. 

Case 2

Incident type

  • Continued inappropriate behaviour despite interventions/ verbally abusive to peers /continued misuse of equipment/ interrupting the learning of others / wandering out of session without permission/Refusal to leave class /threatening peers / blatant challenging of authority / minor damage to property / leaving site without permission

Consequence may include;

  • Parents/Carers informed.
  • Phone call to school. School should support the process of the learner being removed from provision or gaining parental consent for the learner to leave site,  if they cannot be kept on site.
  • The Contribution function can be used on CLM.
  • Incident Report -Level 2 to be complete and submitted to CAP.
  • School will receive a copy of this.
  • A behaviour meeting must be arranged with support of the Monitoring Officers.

Case 3

Incident type

  • Significant incident e.g. physical harm/possession of weapon /possession or supplying substances on site/stealing/any gross misconduct.
  • Continued poor behaviour despite behaviour meetings being held and actions being set.

Consequence may include;

  • Learner to be removed from site immediately (parents and school contacted).
  • Police to be contacted if this is a criminal offence. In any circumstances where the learner becomes a danger to themselves or others the police MUST be contacted.
  • CAP must be informed with immediate effect.
  • Incident form completed; statements from others with support from CAP.
  • Meeting with all parties if appropriate; formal withdrawal and written notification to parents

The above consequences are examples only and each case must be investigated and actioned accordingly.
